Tile is a finish. It is not the structure that should carry a person who grabs the bar for support.[5] For a stud-mounted grab bar, the fasteners pass through the tile or wall panel and engage a stud or solid blocking behind it.[4][5] If a manufacturer allows a tested hollow-wall anchor, use only the approved anchor, wall assembly, and installation method for that exact bar.[4]
A grab bar can sit flat against the wall and still be mounted badly. Tight screws and uncracked tile do not tell you what is carrying the load behind the finished surface.
The useful question is simple: what does each mounting flange fasten into?
Tile hides the support. It does not replace it.
A tiled shower wall may include tile, setting material, backer board, and framing. An acrylic or fiberglass surround sits in front of studs or added blocking. These surface materials finish the wall and help manage water. They are not interchangeable with structural backing.[5]
Kohler’s instructions for one grab-bar line separate those jobs. The installer drills through the ceramic tile, then mounts the wall plates to bracing between the studs. Kohler also says to install that bracing before the finished wall material goes up.[5]
Moen gives similar instructions for one of its SecureMount grab-bar systems. Its standard stud-mounting directions say the screws must enter a wood stud, and both ends must sit over a stud unless the installer uses the specified SecureMount anchors.[4]
Those instructions apply to the products they name. They also show why a universal answer about “anchors” is not enough. The bar, fasteners, backing, and wall assembly have to work as one tested installation.[4][5]
What carries the load?
A towel bar holds towels. A grab bar may steady someone stepping over a tub wall, turning on a wet floor, lowering onto a seat, or reacting to a sudden loss of balance.
The load travels through five parts:[3]
- the bar;
- the mounting flange;
- the screws or manufacturer-approved anchors;
- the backing behind the finished wall;
- the wall structure.
The U.S. Access Board’s ADA guide uses a 250-pound vertical or horizontal force at any point on the grab bar, fastener, mounting device, or supporting structure as its structural-strength benchmark.[3] That number is a technical reference, not a claim that every private bathroom must follow every ADA design rule.
For a homeowner, the lesson is narrower. The metal bar is only one part of the system. A weak connection anywhere between the bar and the wall can defeat the whole installation.[3]
Studs and blocking solve different problems
Studs provide solid framing at fixed locations. If both mounting flanges align with suitable studs and the manufacturer’s instructions allow that position, the fasteners can engage the framing directly.[4]
Blocking is solid material installed between studs. It lets the installer put structural support where the user’s hand needs the bar, rather than moving the bar to whichever stud is easiest to reach. Blocking is usually easiest to add while the wall is open during a shower conversion or remodel.[5]
Placement comes first. The related NextStep guide, Where Should Grab Bars Go in a Tub-Shower?, starts with entry, standing, turning, and sitting movements. It also notes that the wall assembly and mounting system must support the selected location.[2]
Stud mounting and approved anchors are not the same thing

A hollow-wall fastener that works for a mirror, shelf, or towel bar is not automatically suitable for a grab bar. Product instructions control. Do not mix a bar from one manufacturer with an anchor system that the bar manufacturer has not approved.[4][5]
Six questions to ask before installation
- What will each mounting flange fasten into?
- Do both ends have the support required by the bar manufacturer?
- If an anchor is proposed, is it approved for this exact bar and wall assembly?
- How will drilled penetrations be sealed against water?
- Does the location fit the person who will use the bar?
- Could the drilling path reach plumbing, wiring, or the shower’s waterproofing system?
“The screws are in the tile” does not answer the first question.
Warning signs after a bar is installed
Have the installation checked if the bar:
- moves, rotates, or clicks at the wall;
- has a flange pulling away from the surface;
- sits beside cracked grout, loose tile, or damaged sealant;
- was mounted with ordinary plastic drywall anchors and no product-specific approval;
- was placed only where drilling was easy, without checking the user’s reach.
Do not test a questionable bar by hanging your full weight on it. Movement or uncertainty is enough reason to stop using it until someone identifies the mounting method.
Plan backing before the wall closes
A remodel gives the installer a clean chance to coordinate placement and structure. With the wall open, the installer can map likely bar locations, add blocking, and preserve options for a future seat or another support point. Kohler’s instructions use this sequence: install bracing between the studs before installing the finished wall material.[5]
You do not have to install every possible bar at once. Adding backing while the wall is open can make a later installation simpler without forcing the final location today.

Frequently asked questions
Can tile hold a grab bar by itself?

Does a 250-pound ADA benchmark apply to every private bathroom?
No. The U.S. Access Board figure is a structural benchmark from the ADA Standards. It does not mean every private home must follow every ADA layout requirement.[3]
